Please bring your health insurance identification card. If your insurer requires a referral, make sure you have obtained that referral and, if necessary, bring a copy with you.

Be prepared to pay your health insurance co-pay on the day of your visit.


Bring all of your prescription insurances and discount cards; these can include: EPIC, Medicare-D, and PACE.

Roswell Park patients receive two separate bills for service, one for hospital services from Roswell Park Comprehensive Cancer Center and one from The Roswell Park Clinical Practice Plan for physician/professional services. Pay your bill online.

Financial Assistance
