Our Care Conversation series is for patients and caregivers to support, encourage and educate. The format is a 45-minutes presentation followed by 15-minutes for Q&A.
Cancer Survivorship and the Survivorship Care Plan will be led by Jason Rein, MSN, BSN, RN, NP, in Survivorship and Supportive Care.
This session will cover topics in relation to:
- Cancer survivorship plans
- Services available through our Surivovrship Program
- Long-term effects of cancer treatment
- Tips for staying healthy after cancer treatment
