• A personalized Survivorship Care Plan that outlines your care going forward, based on your unique diagnosis, treatment history, risk for secondary cancers or other conditions — and your personal wellness goals.
• Annual visit with a survivorship provider for medical and preventive care
• Cancer screenings and surveillance scans
• Symptom management to address any new or ongoing symptoms of fatigue, pain, trouble with sleep and more
• Mental health support and guidance
• Legal and financial assistance
• Nutrition counseling, help with fertility, smoking cessation and much more!
