Your survivorship plan — a roadmap for what's next

Someone's places their hand on another person's shoulder

This Care Conversation is part of a monthly speaker series at Roswell Park.

Knowing how to best live a healthy lifestyle after you’ve completed cancer treatment can be complicated and your mind may be filled with questions, but Roswell Park’s Survivorship Program is here for you. The comprehensive program brings together many of Roswell Park’s clinical and supportive services, specifically designed for people who are facing a new phase of their cancer journey: survivorship.

So, what is cancer survivorship? And, what services might be beneficial to you? We invite you to find out more at the next Care Conversation, “Cancer Survivorship and the Survivorship Care Plan” on Tuesday, May 21, from 11 a.m. to 12 p.m.

Presented by Jason Rein, MSN, BSN, RN, NP, a Nurse Practitioner in Roswell Park’s Survivorship & Supportive Care Department, you will learn about how a Survivorship Care Plan can help you stay healthy. You’ll also find out what is included in this plan, how to stay on track with recommended screenings and some basics on what to expect for a schedule of visits back to Roswell Park now that you’re done with your treatment. Rein will also discuss long-term effects of cancer treatment and tips on how to stay healthy.
