PACT is currently serving all patients in Erie County.

PACT is a program for Roswell Park Comprehensive Cancer Center and the Roswell Park Care Network, aiming to provide seamless transitions and quality service to you and your caregivers.

Our providers will perform a thorough assessment in the home and establish a plan of care that best meets your health care needs. The following services are available to you:

  • Preventative healthcare, oncology specialty care
  • Medication reconciliation
  • Complex symptom management and palliative care
  • Diagnosis and treatment of medical conditions
  • Prescription of medications
  • Support to family members and other caregivers
  • Referrals for rehabilitation, nursing, hospice and other home-based services
  • Assistance with practical needs, including wheelchairs and other durable medical equipment
  • Care for patients with specific needs to facilitate appropriate discharges

PACT is currently serving all patients in Erie County.

The PACT program is designed to:

  • Prevent readmission to the hospital by providing continuity of care
  • Reduce and prevent emergency room visits and hospital admissions
  • Triage patients more effectively from hospital to home
  • Provide home-based management of cancer and treatment symptoms

Our Why

Roswell Park Community PACT Program is a program of Roswell Park Comprehensive Cancer Center. We believe home-based care clinicians have an obligation to provide and utilize the most efficient and effective means of meeting the patient’s physical, social, emotional, and spiritual needs. This program advances this goal by meeting the care needs of our patients in their homes, outside of the traditional clinical or hospital setting. We aim to provide education, treatment and community resources to our patients and caregivers to allow them to make informed healthcare decisions, manage chronic and severe illness, and address high-risk factors to prevent hospitalization.
