Who We Are:

The Case Management Department at Roswell Park Comprehensive Cancer Center.

How We Can Help:

Our Nurse Case Managers assist in referring patients for skilled home care services such as home nursing, home infusions, and other services you may need at home after you leave the hospital.

What You Need To Do:

Nurse Case Managers are assigned to you while you are in the hospital. Otherwise, your doctor needs to order skilled home care services for you, which you can request from your doctor at your next visit. You must have a skilled need to be considered; home health aide services are requested through the Department of Social Work. Not all patients qualify for home health care. Coverage and available services or equipment depends on your insurance, your specific skilled needs, and the services available in your home county.

The Department of Case Management is available and onsite:

  • Monday through Friday from 8:00 a.m. to 5:00 p.m.

Please call the Department at 716-845-5735 from 8:30 a.m. to 4:30 p.m. Monday through Friday.
