Hematopathology

The Division of Hematopathology includes the bone marrow, cytogenetics, and flow cytometry laboratories, creating a single unified service. We provide comprehensive diagnostic reports that integrate complex information from morphological assessment, immunohistochemistry, flow cytometric analysis, cytogenetic, and molecular genetic studies.

With six faculty hematopathologists focused on oncology, our division has significant expertise across all types of hematological malignancies. Our cytogenetic and flow cytometry laboratories are internationally recognized for their expertise in clinical diagnosis and their innovations in cutting-edge technologies. 

Bone marrow laboratory

The bone marrow laboratory is the central location for the retrieval and processing of specimens collected in the inpatient and outpatient clinical setting. Specimens include bone marrow, peripheral blood, body fluids, and slides for lymphoma studies. Preparation includes smearing, staining (Wright-Giemsa), and screening morphological review by bone marrow technologists. Services are available 7 days a week for preparation of specimen to review by a hematopathologist.

Cytogenetic laboratory

The Clinical Cytogenetics laboratory offers conventional cytogenetic (karyotype) analysis of hematologic malignancies and solid tumors for the Leukemia, Lymphoma, Myeloma, Transplant and Cellular Therapy, Pediatric and Pathology services. Tissues analyzed include peripheral blood, bone marrow, lymph node, effusions, and tumor tissue. Services include:

  • Fluorescence in situ hybridization (FISH) analyses for metaphase and interphase preparations, touch preps, and formalin-fixed, paraffin-embedded tissues.
  • Plasma cell enrichment
  • Fibroblast culture for genomic analyses

Flow cytometry laboratory

The Clinical Flow Cytometry Laboratory at Roswell Park Cancer Center is a CLIA licensed clinical laboratory regulated and certified by the New York State Department of Health, CAP and JCAHO. The laboratory has been in continuous operation performing flow cytometry testing since 1986. The flow cytometry laboratory is equipped with state-of-the-art 8 and 10-color flow cytometers. In 2020, the laboratory performed approximately 8,746 tests, that included cases for Clinical (5,980), QA (2,196) and Research (570). The laboratory offers an extensive test menu:

  • Minimal residual disease testing for acute myeloid leukemia, precursor B-lymphoblastic leukemia (COG), plasma cell myeloma, and mature B-cell lymphoproliferative disorders.
  • Immunophenotyping for hematopoietic malignancies (lymphoma, leukemia, myeloma, etc)
  • T-lymphocyte, B cell and NK cell subset evaluation
  • Paroxysmal nocturnal hemoglobinuria (PNH) • Autoimmune lymphoproliferative syndrome (ALPS) • Standard of Care (SOC) immune reconstitution profiling
  • Stem cell enumeration (CD34 cell count)
  • CMV T-cells (Dextramer)

The flow cytometry laboratory actively performs flow cytometry testing related to clinical trials, experimental studies, and research. In addition, the laboratory participates teaching of medical students and graduate students and provides clinical training for hematopathologists and medical professionals around the world. Flow and Image Cytometry Shared Resource

The Flow and Image Cytometry Shared Resource (FICSR) is one of 15 shared resources supported by the Roswell Park Comprehensive Cancer Center Support Grant (CCSG) from the National Cancer Institute. The FICSR provides advanced flow cytometric analysis and sorting services, as well as imaging services at subcellular levels of resolution in a cost-effective and scientifically rigorous environment with a major strategic goal to provide custom methods development for the technology provided.         

The resource supports both basic research and clinical protocol services and the quality assurance program for all flow cytometers. The resource follows regulatory agency guidelines including those of the New York State Department of Health (NYSDOH), the College of American Pathologists, the Joint Commission on Accreditation of Healthcare Organizations (JCAHO), and the Clinical Laboratory Improvement Act. The resource serves as the central reference flow cytometry laboratory for national and international multi-institutional studies including those of the National Donor and Marrow Program as well as Industry-sponsored studies.        

Contributions and significance to the number of investigators served is evidenced by 266 publications dependent on the FICSR services that were published between 2016-2021 of which 70 were co-authored by FICSR personnel. During that same time, Roswell Park-wide, 870 grant applications were submitted relying on the FICSR.
