Let’s face it. Healthcare is a complex system and navigating its “paperwork” – determining what forms and who completes them — is often confusing. Here we answer some frequently asked questions from our patients:

How do I obtain a copy of my medical records?

To request your medical records from Roswell Park, contact the Medical Records department at 716-845-5990. Or you can download the Request Form and print. Once you fill out the form which asks you what records you want, where they should go and how you want them delivered, you can either:

  • Fax the form to: 716-845-8394, or
  • Mail the form to: Roswell Park Comprehensive Cancer Center, c/o Health Information Management, Elm & Carlton Streets, Buffalo, NY 14263

Requests will be processed in 7-10 days of receipt. Please note there may be a copy charge. If you have any questions, please call 716-845-5990.

To obtain copies of radiology images or films, please contact the Roswell Park Film Room directly at 716-845-3209.

I am requesting disability benefits, who should I contact?

Some patients may be unable to work while they are receiving treatment. It is important to talk to your doctor about how your treatment may affect you. Disability benefits can help make up for part of your lost income if you are unable to work through treatment. Disability benefits may be obtained through your employer, New York State or Social Security, depending on your situation, or through disability insurance if you have it. Benefits can vary from employer to employer, so it is important to talk to your employer about what may be available to you.

Social workers at Roswell Park can help guide you through your options and how to apply for benefits. You may reach the Social Work department at 716-845-8022.

Staff in the Roswell Park Disability Office on the ground floor of the main hospital also are available to assist you with questions you may have. If you need help with disability insurance forms, call the office at one of the phone numbers below, depending on your last name:

How do I submit disability forms for my provider to fill out?

After you obtain the proper disability forms from your employer or your disability insurance and complete your part of it, you’ll need your Roswell Park physician to complete the provider portion. Every clinic has a drop-off box for disability forms, or you can give them to your clinic’s front desk assistant. You can also submit them by:

  • Dropping your forms into the white Drop Box in the hospital lobby behind the New Patient Registration desk
  • Fax to: 716-845-3896
  • Email to: HIMDisabilitySpecialists@RoswellPark.org
  • Message via MyRoswell Patient Portal
  • Mail to: Roswell Park Comprehensive Cancer Center c/o Disability Office Elm & Carlton Streets Buffalo, NY 14263

Turnaround time is approximately 7 to 10 business days.
