Modify a License/Registration

After initial approval of your New York State Bureau of Narcotic Enforcement controlled substance license and Drug Enforcement Administration (DEA) registration, common modifications include:

  • Adding/deleting substance schedules
  • Address changes (e.g., moving labs and storage locations within Roswell Park or transferring to a different academic institution)
  • Name changes

Submit modifications

Submit requests to modify your license/registration first to New York State then, upon approval, to the DEA.

New York State Bureau of Narcotic Enforcement License

Licensee changes including official name, address or ownership must submit a new DOH- 4330. Enter current and new information. Requires inspection. A new BNE license number may be issued. May require facility inspection by BNE.

All NYS BNE researcher license modifications must include the following:

  1. A completed DOH 4330 form
  2. A completed Appendix B (Institutional Researcher (Schedules I-V)) or a completed Appendix A1 (Individual Researcher (Schedules I-V))
  3. Copy of DEA registration as a Researcher or copy of Practitioner DEA registration if DEA deems research is incidental to a Practitioner’s practice
  4. All other items from applicable new application (Class 4 or Class 7 Institutional/Individual Researcher) that have changed or been updated
  5. A short description listing the information to be updated (e.g., room number, last name) and digital photographs of the intended storage, if changing the physical security of controlled substances
  6. Check or money order made out to the NYS DOH Bureau of Narcotic Enforcement

DEA Registration

Upon NYS BNE approval, submit changes to DEA registration online.

Changes involving Schedule I and II substances also require new DEA 222 order forms.

Moving storage locations

Within Roswell Park

When changing storage locations within Roswell Park, do not move the controlled substances until the DEA approves the new location and, if applicable, you receive new DEA Form 222s. The following steps should be used as a guide for moving:

  • Dispose of all expired or unwanted controlled substances.
  • Conduct the following inventories:
    • A closing inventory at the previous location
    • An initial inventory at the new location
  • Keep all records and completed DEA Form 222s from the previous location for two (2) years.

If applicable, mail unused DEA Form 222s to the DEA (see below for the mailing address)

Transferring outside Roswell Park

Licensees/registrants who leave the employment of Roswell Park cannot transfer the controlled substances under their purview to a new location outside of Roswell Park.

  1. Dispose of all expired or unwanted controlled substances.
  2. Conduct a closing inventory at the U-M location.
  3. Keep all records and completed DEA Form 222s (if applicable) from the Roswell Park location for two (2) years.

If applicable, mail all unused DEA Form 222s to the DEA.

If transferring within New York State, you must reflect the new affiliated institution and research activities on your controlled substance license and registration by renewing your NYS BNE license, then modifying your DEA registration online. If transferring out-of-state, apply for a controlled substance license in the U.S. state/territory where the new institution is located and retire your DEA registration for Roswell Park.

Retire or cancel a research license/registration

You cannot transfer controlled substances to another Roswell Park researcher or leave them in the storage location for others to use for research when retiring or cancelling your license/registration. Always:

  1. Dispose of all controlled substances under your registration.
  2. Conduct a closing inventory

For New York State

You do not need to notify the NYS BNE to retire your license. The license will lapse when not renewed.

For the DEA

Email a notice of retirement to DEA.Registration.Help@usdoj.gov, and include your name, address, phone number, DEA number, effective date and reason for cancellation. If applicable, mail unused DEA Form 222s to the DEA.
