Description of Duties:
The Clinic Liaison Nurse Practitioner develops and maintains new and current relationships with hospitals and other healthcare entities within our region. The Clinic Liaison Nurse Practitioner works closely with physicians and other clinical staff, social workers, case manager, discharge planners and administrators to ensure optimal care of Roswell Park patients utilizing the healthcare services provided in other community healthcare settings.
Applicants must have a License to practice as a Registered Nurse in the State of New York, be a Current Nurse Practitioner certification in New York State; and have Certification as a Nurse Practitioner by the American Nurses Credentialing Center or other national certifying body recognized by Health Care Financing Administration (HCFA). They must also have a Master’s degree in Nursing and the equivalent of one year of full-time experience in one of the following environments: community practice, community health liaison, oncology, or acute care.
