Description of Duties:
The Clinical Pharmacist – Specialist assumes responsibility for medication management for a core group of patients including prospectively evaluating all drug therapy for appropriate indications, dosage, drug interactions, and drug allergies. The Clinical Pharmacist is also responsible for monitoring patient's pharmacotherapeutic regimen for effectiveness and adverse drug events and intervening as needed; providing medication reconciliation and patient education. The Clinical Pharmacists also acts as a liaison between pharmacy, nursing, and the medical staff to educate health professionals regarding current drug related procedures, policies, guidelines and pathways; participating in quality assurance programs to enhance pharmaceutical care. There will be ample teaching opportunities available such as precepting PGY2 oncology specialty residents and pharmacy students, and providing educational lectures for medical and pharmacy staff. This position also requires involvement in Clinical Research - Clinical Pharmacy Services for inpatient and Ambulatory patients.
Applicants must have a License to practice as a Registered Pharmacist in the State of New York as well as a Doctor of Pharmacy degree; or a Master’s degree and the equivalent of one year of full-time clinical pharmacy or pharmacy-related experience; or a Bachelor’s degree and the equivalent of three years of full-time clinical pharmacy or pharmacy-related experience.
Incumbents must be credentialed as a member of the Institute Medical Staff (Allied Health Professional) within four months of date of hire as a condition of continued employment.
The preferred candidate will have previous experience in a hospital pharmacy, experience in an oncology setting and be Board Certified in Oncology.
