Senior Case Manager
The Senior Case Manager coordinates care of assigned patients throughout the continuum of care, including the outpatient cases and linkage with appropriate community services; assesses appropriate level of care, communicates with patients, families, medical staff, community agencies and third party payors to ensure efficient and appropriate care and resource utilization; utilizes assessment, planning, implementation, coordination, monitoring and evaluation skills in the process of developing and executing a plan of care for the patient; evaluates new treatments and technologies and the cost/benefit of these to the patient; acts as an advocate for the patient and a point of contact for third party payors in assessing the need and benefit of services/care for the patient; supervises case management staff; evaluates and oversees performance standards for assigned staff; coordinates programs, initiatives and performance improvement under the direction of the supervisor and manages the department in the supervisor’s absence.
Applicants must have a License to practice as a Registered Nurse in the State of New York. The applicant must have a Bachelor’s degree in Nursing and the equivalent of three years of full-time clinical experience in the field of utilization management/discharge planning, oncology, home health care, case management or a related field. The preferred candidate will have two years of experience in a supervisory role, utilization management experience, denials management experience, and discharge planning experience. They will also be a Certified Case Manager, will have managerial skills, will be a team player, and good communication skills.
