The Case Manager coordinates care of assigned patients from admission to discharge and post-hospital care; assesses appropriateness of level of care, communicating with patients, families, medical staff and third party payers as necessary to ensure effective patient care and resource utilization.
The applicant must have a License to practice as a Registered Nurse in the State of New York. They must have a Bachelor of Science Degree in Nursing and the equivalent of two years of full –time clinical experience in the field of Utilization Management/ Discharge Planning, Home Health Care or Case Management; or an Associate’s degree in Nursing and the equivalent of four years of full-time clinical experience in the field of Utilization Management/Discharge Planning, , Home Health Care or Case Management or a Nursing diploma and the equivalent of five years of full-time clinical experience in the field of Utilization Management/Discharge Planning, Home Health Care, Case Management. The preferred candidate will have utilization management experience, denials management experience, discharge planning experience, and experience working in a hospital setting. They will also be a Certified Case Manager, a team player and will have good communication skills.
