Description of Duties:
The Case Manager coordinates care of assigned patients from admission to discharge and post-hospital care. The Case Manager also assesses appropriateness of level of care, communicating with patients, families, medical staff and third party payers as necessary to ensure effective patient care and resource utilization.
Applicants must have a License to practice as a Registered Nurse in the State of New York. they must also have a Bachelor of Science Degree in Nursing and the equivalent of two years of full–time clinical experience in the field of Utilization Management/ Discharge Planning, Home Health Care or Case Management; or an Associate’s degree in Nursing and the equivalent of four years of full-time clinical experience in the field of Utilization Management/Discharge Planning, Home Health Care or Case Management or a Nursing diploma and the equivalent of five years of full-time clinical experience in the field of Utilization Management/Discharge Planning, Home Health Care, Case Management. The preferred candidate will have their CCM certification, experience in the Case Management arena, in assessing and coordinating Post Hospital Care Needs/Discharge Plans, in collaborating with Multidisciplinary Care Teams, and in working with community agencies. They will also have experience in conflict resolution, experience working with payers, knowledge of Utilization Review Activities/Tools, knowledge of levels of care, of LOS barriers/Impact of LOS, and knowledge of the Health Care Delivery System.
