If you’re facing a cancer diagnosis, we want you to know that we share your concerns about taking action quickly. We’ll guide you through the first steps you need to take in scheduling a consultation and planning your treatment.
To begin, you’ll want to schedule a consultation with our team. You can start either by filling out the Become a Patient Form, or by calling us at 1-800-ROSWELL (1-800-767-9355).
One of our PAT associates will call you to gather some basic details and request the records that document your diagnosis. During this call, we will:
- Gather your name, address, phone number, date of birth, Social Security number and insurance information. We’ll also ask for contact information for your referring physician and any other providers you may be working with. We may ask you for further details about referrals or authorizations.
- Ask you to make sure that, before your consultation, we receive all of the medical records associated with your diagnosis. We’ll need to review such documents as the results of CT, PET, or bone scans, MRIs and other tests, and pathology reports before your consultation. If you like, we’ll explain how you can arrange to have these materials sent to us.
- Set up an appointment for your consultation, and suggest that you bring a family member or friend with you when we meet. Ordinarily, your consultation will take place about seven days after your first conversation with our Patient Access Team.
- After we set up your appointment, we’ll mail you a packet containing a confirmation of the date and time of your appointment, and useful information about Roswell Park.
Arranging for a Second Opinion
Most physicians welcome second opinions, and in most cases, a second look by experts will confirm your initial diagnosis and your doctor’s plan. At Roswell Park, most patients who need a second opinion are seen within a week. To arrange for a second opinion, call 1-800-ROSWELL (1-800-767-9355) or fill out the online Become a Patient Form.
