The Clinical Pharmacist - Specialist assumes responsibility for medication management for a core group of cancer patients (specific therapeutic area) including prospectively evaluating all drug therapy (especially antineoplastic therapy) for appropriate indications, dosage, drug interactions and drug allergies; monitoring patient's pharmacotherapeutic regimen for effectiveness and adverse drug events (ADE) and intervening as needed; providing medication reconciliation and patient education; acting as a liaison between pharmacy, nursing and the medical staff to educate health professionals regarding current drug-related procedures, policies, guidelines and pathways; participating in quality assurance programs to enhance pharmaceutical care. Will also be responsible for developing and implementing pharmacy programs and initiatives. There will be ample teaching opportunities available such as precepting PGY2 oncology specialty residents and pharmacy students, and providing educational lectures for medical and pharmacy staff. This position also requires involvement in clinical research - Clinical Pharmacy Services for inpatient and ambulatory patients.
Minimum qualifications:
License Requirement
License to practice as a Registered Pharmacist in the State of New York
Education and Experience
1. Doctor of Pharmacy degree; or
2. Master’s degree and the equivalent of one (1) year of full-time clinical pharmacy or pharmacy-related experience; or
3. Bachelor’s degree and the equivalent of three (3) years of full-time clinical pharmacy or pharmacy-related experience.
NOTES:
Required degrees must have been granted by an accredited college or university or one recognized by Roswell Park Cancer Institute as following acceptable educational practices.
Incumbents must be credentialed as a member of the Institute Medical Staff (Allied Health Professional) within four months of date of hire as a condition of continued employment.
Preferred qualifications:
The preferred candidate will have previous experience in a hospital pharmacy, experience in an oncology setting and be Board Certified in oncology.
