2. Complete form and return to Purchasing Dept. located on the 6th floor of the C&V Building.
3. Provide the following information to the Supply Center Staff:

Last Name

First Name

Email Address

Phone Number

PI Name

Cost Center/Grant Number(s) (This will appear as "Account Number" in the system)

Your account will take ~48hrs to be validated by Invitrogen at which point you will receive a confirmation email containing your username and password.
