Eligibility Criteria:

Applicants must possess the following:

License Requirement:
License to practice as a Registered Pharmacist in New York State.

Education and Experience:
1.Doctor of Pharmacy degree and 2 years of full-time clinical pharmacy or pharmacy-related experience; or
2. Master's degree and 3 years of full-time clinical pharmacy or pharmacy-related experience; or
3. Bachelor's degree and 5 years of full-time clinical pharmacy or pharmacy-related experience.

Note:  Incumbents must be credentialed as a member of the Institute Medical Staff (Allied Health Professional) within four (4) months of date of hire as a condition of continued employment.

The preferred candidate will be a Board Certified Oncology Pharmacist (BCOP), possess a PharmD degree, and have completed an accredited clinical residency program.  5 years of clinical pharmacy experience in a hospital setting and previous experience as a Clinical Coordinator or Director in a large, complex facility are also preferred.

Nature of Work and Responsibilities:

This Clinical Pharmacy Service Director will develop, expand, provide and direct clinical pharmacy services including supervision of Clinical Pharmacists.  This candidate will maintain standards set by the Department and regulatory agencies in the establishment and leadership of high quality clinical services for optimal patient outcomes.  Further duties will include being responsible for the formulary management and budgetary reporting, as well as providing drug information services and education for pharmacy and patient care services staff.
